Your scheduled appointment time has been reserved specifically for you. We request 24-hours notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment. After missing your second appointment without notifying us 24 hours in advance, you are subject to being charged an additional fee. Short notice cancellations result in a delay of treatment, as we book appointments one month in advance. The sooner you know an appointment needs to be changed, the sooner it can be rebooked.
The orthodontist has a direct relationship with the patient. The patient has a direct relationship with their employer, and in turn, the employer has a relationship with the insurance carrier.
Your insurance company will require that a treatment plan be submitted for the authorization of benefits. We will supply you with the proper form at the examination or recall exam outlining the treatment and fees. You will then need to submit it to your insurance carrier. The insurance company will not start to reimburse you for the treatment until they have authorized it.
There are many types of insurance plans. Most plans do not pay for the entire cost of your care. There usually is a dollar limit (maximum) and sometimes excluded services.
